Privacy Policy

Last updated September 16, 2026

This policy explains what information the nesivosbh.com website collects, why, who receives it, and the choices you have. It covers this website only. Health information you share with us as a patient is protected under HIPAA and handled according to our Notice of Privacy Practices, not this policy.

Information you give us

When you complete our contact form we collect what you enter: your name, email address, phone number, your relationship to the person seeking care, and any message you write. We use it only to respond to you and, if you wish, to arrange care.

Please do not include medical details in the website form. A web form is not the right place for clinical information. Send us your contact details and we will follow up with you directly.

Form submissions are emailed to our team and kept in the website’s database so we can respond and keep a record of inquiries.

When you request an appointment, the details you submit are also saved to a secure spreadsheet that our staff use to schedule your appointment.

Information collected automatically

Analytics, only if you allow it

If you choose Accept analytics, we use Google Analytics to understand which pages are visited, roughly where visitors are located, which device and browser they use, and which site referred them. We see this in aggregate. We do not use it to identify you, we do not combine it with any patient record, and we do not use advertising or remarketing features.

Until you accept, the Google Analytics code is not loaded at all and nothing is sent to Google for analytics. Analytics never runs on our contact page, even if you have accepted.

Spam and abuse protection

Pages where our contact form can be opened load Google reCAPTCHA. reCAPTCHA checks whether a visitor is a person or an automated program by looking at information such as your IP address, browser and device details, and how you interact with the page. We use it only for security and to prevent fraud and abuse. Google processes this information for us as our service provider.

Server logs

Like every website, our hosting server records basic technical information about each request, such as IP address, browser type, and the page requested, to keep the site secure and working.

Cookies and your choices

Our cookie policy lists every cookie this site uses, what each one does, and how long it lasts. You can change your choice at any time with the Cookie settings link at the bottom of every page.

Global Privacy Control. If your browser sends a Global Privacy Control signal, we treat it as a choice of Essential only: analytics stays off and we do not ask.

Do Not Track. There is no common standard for responding to browser Do Not Track signals, so we do not change our behavior based on them. Analytics is already off unless you turn it on.

Text messages

If we communicate with you by text message about your care, message frequency varies and message and data rates may apply. Reply STOP to opt out or HELP for help at any time. We do not share mobile phone numbers or text messaging opt in data and consent with any third parties or affiliates for marketing or promotional purposes.

Who we share information with

We do not sell your information and we do not share it for advertising. We share it only with service providers who help us run this website and respond to you, and only as needed for that purpose:

  • Google Analytics, for website statistics, only if you accept analytics
  • Google reCAPTCHA, to protect our forms from spam and abuse
  • Our website hosting provider, which stores the website and form submissions
  • Our email provider, which delivers form submissions to our team

We may also disclose information when the law requires it, or to protect the safety of any person.

Our website fonts and all other site files are served from our own website, not loaded from outside services.

How long we keep information

We keep form submissions only as long as we need them to respond to you and to meet our record keeping obligations. Analytics data is kept according to the retention setting in our Google Analytics account.

Your choices and requests

  • Accept or decline analytics, and change your mind at any time
  • Ask what information we hold about you from this website
  • Ask us to correct or delete information you sent through this website

To make a request, email info@nesivosbh.com or call 732-641-1751. Requests about your medical records are handled under our Notice of Privacy Practices.

Children

This website is not directed to children under 13, and we do not knowingly collect their information through it. Care for adolescents is arranged with a parent or guardian.

Changes to this policy

If we change this policy, we will update the date at the top of this page.
